Acetabular fixation during revision total hip arthroplasty in patients who have a non supportive
superior dome and proximal migration of the acetabular component (a Paprosky Type–IIIa defect) cannot
be achieved reliably with use of a hemispherical porous-coated component alone. In this JBJS/VJO video
supplement to the article, Dr. Scott M. Sporer, MD and Wayne G. Paprosky, MD discuss and demonstrate
the use of structural trabecular metal augmentation for revision at the site of a Type–IIIa defect. The
material appears to allow extensive bone ingrowth and may ultimately change the indications for the use
of a monoblock acetabular component.
